New Patient Paperwork

When you make your appointment, please be sure to let our staff know if you have insurance so that it can be verified prior to your appointment. The first visit is always the longest. To save yourself some time, please download the New Patient Paperwork.

We try to not over schedule our patients, therefore, we pride ourselves in minimal wait times. We conduct a thorough exam and afterward discuss your findings along with a suggested treatment plan. Often, at a patient’s request and if insurance allows, patients will receive their first treatment the same day of the exam.
